The form Application to modify the lease

Your application pertains to a modification that you want to make to the lease and that was refused by the lessee, and you want the Régie to fix the amount of the rent and/or that the Régie renders a decision on the desired modification.

This application must be filed by the lessor within one month following receipt of the lessee's refusal.

IF YOU WANT TO COMPLETE THIS FORM :

Address of dwelling in question : enter the full address of the dwelling concerned by your application.

Description of parties :
Lessee : person that rents the dwelling in question.
Lessor : person that owns the dwelling in question.

Costs exigible : to file such an application at the Régie du logement, costs are exigible. As of November 1st 2011 these costs are of :

• $42 if the rent is $350 or less;
• $52 if the rent is more than $350 but not more than $600;
• $68 if the rent is more than $600.

Please note : the application cannot be served upon the other party before being filed (stamped) at the Régie du logement.
